First Visit

On your first visit we ask you to fill out a short medical/dental history form. The form is available in PDF for your convenience, but please do not feel obligated to fill it out prior to your visit. We will expose a digital full mouth x-ray unless your dentist has provided a current one. After a thorough examination of your teeth and bite, Dr. McCallum will give you a detailed explanation of his findings and what treatment may be necessary. A treatment coordinator will explain appointments, insurance, payment options, and any other aspects of your treatment. We encourage you to ask questions. Our goal is that you leave our office knowing what treatment is needed, how the treatment will be accomplished, how long it will take, what to do next, and how much it will cost.
